Terms & Policies

Clinic Policies:

Zalum Consulting Firm provides comprehensive psychiatric evaluations, ongoing medication management, solution-focused psychotherapy, and lifestyle coaching for clients aged 9-65. Collaboration between our providers and clients ensures the best outcomes when all parties clearly understand the goals and expectations of service delivery. Please review our policies and procedures carefully to ensure a smooth service experience.

Zalum Consulting Firm does not offer “on-call” availability or treatment outside of regular business hours. If our services do not meet your needs, we will assist you with referrals to more suitable care providers.


Scheduling an Appointment

To schedule an appointment, please complete and submit the intake form (Appointment Request Form). Upon receipt, a staff member from Zalum Consulting Firm will contact you to finalize scheduling.

Prior to your appointment, you will receive access to the Client Portal, where you must complete the required new client paperwork. This paperwork must be submitted at least 5 days prior to your appointment. Failure to do so will result in your appointment being canceled and rescheduled at a later date.


Payment at Time of Service

Payment in full is required at the time services are rendered. We will provide you with detailed information regarding your insurance coverage and our private pay rates before scheduling your appointment. A credit card is required to be on file for all clients.


Confidentiality

We are committed to safeguarding your information. Zalum Consulting Firm is bound by law to maintain the confidentiality of your personal and health information. However, as mandated reporters, we are required by law to report cases of suspected child or elder abuse or any threat of harm to oneself or others.

Clients are required to sign a statement of understanding regarding their privacy rights, which will be available via the Client Portal.


Cancellation and No-Show Policy

Appointments must be canceled with at least one full business day’s notice. For example, if your appointment is on a Monday, it must be canceled by the preceding Friday.

Clients who are more than 15 minutes late for a 30-minute appointment or 20 minutes late for a 60-minute appointment will be considered a “no-show.” The client’s card on file will be charged $50 for missed appointments or late cancellations. Insurance companies do not cover charges for missed sessions, so this fee is the responsibility of the client.

Zalum Consulting Firm reserves the right to terminate services if a client consistently misses appointments or is not actively participating in their treatment goals.


Right to Terminate Services

Zalum Consulting Firm reserves the right to terminate the provider-client relationship under the following conditions:

  • If services are no longer beneficial to the client
  • If another professional or provider may better serve the client
  • If payment is not received
  • If a client misses two consecutive appointments without proper notice
  • If a client is not cooperating with the agreed-upon treatment
  • If a client or their family member/significant other behaves aggressively or disrupts the work environment

Benzodiazepine Policy

Benzodiazepines (such as Xanax, Ativan, and Klonopin) will rarely be prescribed and only in time-limited quantities, due to their potential risks, including dependence and cognitive decline. Zalum Consulting Firm adheres to strict guidelines regarding these medications.


Stimulant Policy

Stimulants, which are commonly prescribed for ADHD, carry risks such as dependence and abuse. As such, Zalum Consulting Firm requires clients to adhere to a stimulant medication agreement, which will be discussed and signed before treatment.


Refill Policy

We typically prescribe medications to cover your needs until your next appointment. Refills are only considered in exceptional cases and must be requested at least 5 business days in advance. We do not process refill requests on weekends or after hours. Clients are responsible for scheduling follow-up appointments to avoid running out of medication.

Controlled substances cannot be refilled by phone and will only be provided during in-office visits. There will be no early refills for controlled substances, regardless of the reason.


Forensic and Court Support Services

  • Disability: We provide a copy of health records for established clients working with disability attorneys. We do not perform disability evaluations.
  • Emotional Support Animals (ESA): We do not assess or provide letters for emotional support animals. However, we can provide documentation to clients who have been with us for at least three months and who have adhered to all scheduled appointments.
  • Parenting Evaluations: We do not perform parenting evaluations or testify in court on behalf of our clients. Our rate for court appearances, if subpoenaed, is $400 per hour.
  • Paperwork: We charge an hourly rate of $50 for all documentation services provided outside of regular treatment sessions.
